· Mobile Number that will be ported-in to T-Mobile.
o If mobile number has already been submitted or ported-in before February 17, 2017, the customer will receive one of the following error messages:
§ If customer has already entered the promotion: The mobile number entered has already been submitted for this promotion.
§ Customer ported-in before February 17, 2017: The mobile number entered does not meet activation eligibility window.
· First and Last Name.
3. Customer accepts legal disclosures of offer and submits and receives the following reminders:
· Please remember that in order for the discount to be applied to your account, you must:
o Port over the same mobile number you used to reserve this code.
