Petco B&M store had a stack of MIR forms in front of where they stock the Litter Genie Plus.
I scanned a copy of the MIR form. Notable terms and conditions:
- Purchase between 08/19/2018 and 09/29/2018
- Postmark by 10/29/2018
- Receive a check up to $24.99
- Mail the form, UPC code, and receipt with item circled
- Limit of one request per person/household
- Form must be original; no copies allowed (i.e. the attached scan is for your reference only; do not print this out and mail it in)
