UnitedHealthcare health plan enrollees and dependents over the age of 18 who register a new account with Doctor On Demand, Inc. between October 17, 2018 and December 31, 2018, will receive an email from Doctor On Demand containing a link to redeem a five dollar ($5) gift card to Amazon.com. You will be required to enter your name and enter UnitedHealthcare as your insurance provider during registration with Doctor On Demand in order to be eligible. Members with HSA plans are not eligible. You will receive a link from Doctor On Demand to redeem the gift card two to four weeks following the end date of the campaign.
